Generate one form containing multiple candidate information

I want to generate a form for recommendation to appoint that contains reasons why the best candidate is recommended and also comments on the candidates that were not successful.
I want the one form to pre-fill information from multiple candidates, and to be sent to senior leadership to approve and show the one form. So that they can review the decision-making reasoning, not just for the preferred candidate.

This was suggested by one New Zealand client - please ask me for more information
