New questionnaire question type: Multi-select checklist with a free-text option

Request to add a new question type to the questionnaire functionality - similar functionality to the Radio List Text question, but to allow for multi-select. This would allow users to expand on the multi-select options (such as by checking an "other" option box - in addition to other boxes - and provide further details about the "other" option they selected).
